$10 Sports Physicals at Central Indiana Orthopedics Zionsville!

We’re proud to be part of the Zionsville community and excited to offer $10 sports physicals throughout July at our new Zionsville office, conveniently located at 625 S Main Street, Suite 200. No matter the sport, we’re here to make the process quick, easy and affordable. 

What You Need to Know

• Cost: $10 (cash or card only)
• No insurance billing
• Walk-ins welcome — or call ahead to schedule
• Please bring your school’s physical form

• Tuesday, July 1: 9:00 AM to 12:00 PM
• Thursday, July 3: 9:00 AM to 2:00 PM
• Tuesday, July 8: 9:00 AM to 11:00 AM
• Wednesday, July 9: 9:00 AM to 12:00 PM
• Thursday, July 10: 9:00 AM to 2:00 PM
• Tuesday, July 15: 9:00 AM to 2:00 PM
• Monday, July 21: 9:00 AM to 11:00 AM
• Wednesday, July 23: 9:00 AM to 12:00 PM
• Thursday, July 24: 9:00 AM to 2:00 PM
• Thursday, July 31: 9:00 AM to 2:00 PM
